This Website Privacy Policy is specific to the use of this Site only. It does not apply to our therapeutic services that you may engage in upon becoming a client of States of Wellness.
WHAT DATA WE COLLECT AND WHY WE COLLECT IT
By visiting this Site, we may collect 1) personal information you provide to us by engaging with the Site or filling out our “Contact Me” form, and 2) website use data that is automatically collected as you use this Site.
DATA YOU PROVIDE
When you utilize our “Contact Me” form, we ask for data such as your name, email address, phone number, preferred method of contact, and where you heard about States of Wellness. This data is collected so we may follow-up with your request for contact from us. Your information collected through our “Contact Me” form is collected through Google Forms and stored on Wellbeing Therapy’s Google Workspace account with whom we have a HIPAA business associate agreement to ensure confidential storage of this personal information.
While our “Contact Me” form has a “message” option where you can write an optional message to us upon our first contact, we ask that you do not include sensitive personal or health information in this form. Your health information will be collected at a later step in our process using our electronic health records system Sessions Health, or our billing records system Alma in compliance with HIPAA. Information you do share in our blank “message” section on our “Contact Me” form is stored along with the rest of your responses to this form through Google Forms and stored on Wellbeing Therapy’s Google Workspace account with whom we have a HIPAA business associate agreement to ensure proper storage of this personal information. Your response to the “message” option on our “Contact Me” form may be uploaded to your electronic health record should you become a therapy client of Wellbeing Therapy.
If you communicate with us by email, we may save these email communications including the contents of your messages, our replies, and your email address. These will remain within our Google Workspace account as well as possibly be added to your health record should you become a therapy client of Wellbeing Therapy.
Any further contact you have with Wellbeing Therapy is beyond the scope of this Website Privacy Policy and you should refer to our Notice of Privacy Practices which applies to our therapy services and can be found on our website.
WEBSITE USAGE DATA
This Site automatically collects information as you browse. This data is collected in order to enable website features, ensure functionality, and gather statistical information about how website visitors use this Site. We then use this information to better understand how our visitors use this Site.

Any further contact you have with Wellbeing Therapy as a client is beyond the scope of this Website Privacy Policy and you should refer to our Notice of Privacy Practices which applies to our therapy services and can be found through in the portal on Sessions Health.
